Look at top of last page in EM - if software version is 3.5L... then use Langley (NTL) bytes; if 3.5B... then use Bromley (exCW) bytes. You'll have jtag attached to do this so set the netid at the same time - and remember that if you've cleared the netid then you won't be able to read the BK...

If you have an unlooper you could try opening the card first then reading it in nagra, or failing that remove the atmels from the board and read them with a willem.

I've had the same symptoms as you and always find the cure is to clean the legs and reseat the chip in the programmer. Normally finds a cotton bud soaked in meths does the trick.
