Contact Us Website Script

Hi

Does anyone have or does anyone know of a website where I can get a script to produce a contact us web page. The one where the customer inputs their relevant info. eg name, address, tel no, etc and when they click submit and the form is sent to an email address???

any help would be appriciated.

BEWARE. McAfee found a trojan once i had installed this program. And also, this program added some favourites in my webbrowser, without asking me.
