Illusionist David Blaine will leave his glass box live on Sky one tonight (sunday)
The 30-year-old has spent the past 44 days without food or contact in the box, which is suspended over the River Thames near Tower Bridge.
Today a nutrition expert warned he could die when he begins eating again after leaving his confinement if not monitored closely.
"There is a need for careful nutritional and clinical management of David Blaine after the fast," said Professor Marinos Elia from the University of Southampton.
"Not only because of the specific symptoms he has experienced during the fast, but also because inappropriate feeding can precipitate serious problems including sudden death (the re-feeding syndrome)."
Blaine will be lowered to the ground at around 9.30pm Sunday evening during Sky One's live David Blaine: Above the Below exit show.
"He will then be put on a stretcher and taken off in an ambulance to a private hospital," a spokesman said. "He could be in hospital for a few days or more than a week."
