An ambulance was delayed from responding to an 999 call after its driver was told to pay to use the M6 Toll motorway.
Staffordshire Ambulance Service has called for an investigation after the incident at Weeford near Lichfield on Wednesday night.
A spokesman said the paramedic was refused free entry as his vehicle did not have "ambulance" on its side.
He was only allowed through the toll after turning on its blue lights.
