You need a Phoenix programmer & look for Wolverine's tutorials. They're easy to follow & make good reading.
You can get all channels except PPV on a MOSC unless you program card with PPV IDs. This tutorial is on Crows site

You have to modify PBM in eeprom with MKFv4.3 software. As per Gandalf2k's advice but just to add that an Elvis programmer is capable of phoenix mode, as are some others but not all!
